Who We Help
We serve ID/DD disabled persons and their families living in Allegany County, Anne Arundel County, Baltimore City, Baltimore County, Carroll County, Fredrick County, Garrett County, Harford County, Howard County, and Washington County
Featured Services
Residential: Support ID/DD disabled persons in developing and maintaining skills related to activities of daily living (ADLs), instrumental activities of daily living (IADLs), and socialization. Skills are based on the person’s individualized goals and outcomes as documented in a person-centered plan (PCP). We provide:
- Support necessary to live meaningful lives and integrate into communities of preference and choice.
- Formal teaching methods such as systematic instruction.
- Participants with opportunities to develop skills related to activities of daily living.
Nursing Support: A registered nurse, licensed in the State of Maryland performs Nursing Consultation, Health Case Management, and Delegation services, based on the participant’s assessed needs. Initial nursing assessment includes:
- A review of the participant’s health needs, including health care services and support the participant currently receives.
- Participant’s health records, including any physician orders performance of a comprehensive nursing assessment.
- A clinical review of the participant’s Health Risk Screening Tool (HRST).
Nursing Support Services provide for the completion of the Medication Administration Screening Tool, in accordance with DDA policy
Personal Support: Building skills needed in which participants integrate, engage, and navigate their lives at home and in the community.
- In-home skills include development in budgeting and money management, completing homework, maintaining a bedroom or home, cooking, personal care; house cleaning/chores; and laundry.
- In-community skills include the development of skills that make it possible for participants to grocery shop, get a haircut, use public transportation, attend school or social events; join community organizations or clubs, recreate and/or participate in leisure activities, volunteer, and participate in organized worship or spiritual activities of their choice.
Community Development: Community-based activities under this service will provide the participant access and support to engage in community-based activities for the development, acquisition, and maintenance of skills to increase the participant’s independence related to community integration with individuals without disabilities. Community Development Services include:
- Promoting growth and developing general skills and social supports necessary to gain, retain, or advance competitive integrated employment opportunities
- Learning socially acceptable behavior; and learning self-advocacy skills.
Respite Care: Relieves families or primary caretakers from their caregiver responsibilities while providing participants with new opportunities, experiences, and self-determination. Respite care can be provided in
- Participant’s home
- Respite care provider’s home
- A licensed residential site
- A state-certified overnight or youth camp and
- Other settings and camps as approved by DDA
