DDA Nurse

Job description

The DDA-Registered Nurse is responsible for overseeing and managing the healthcare services provided to individuals with developmental disabilities. This includes ensuring that all individuals receive appropriate medical care, coordinating with medical professionals, and managing all aspects of a delegating nurse regarding overseeing Certified Medication Technician.

Responsibilities:

– Utilize your expertise in case management to assess, plan, implement, monitor, and evaluate patient care

– Collaborate with healthcare professionals to develop and implement individualized care plans

– Conduct patient assessments and provide ongoing monitoring of their condition

– Coordinate and facilitate communication between people supported, staff, families, and healthcare providers

– Utilize your knowledge to interpret diagnostic evaluations and develop appropriate treatment plans

– Provide education and support to people we support, staff and their families regarding their healthcare needs

-Assure that nursing case delegation and management provided by nursing is within standards of COMAR 10.27.11, 10.22, and is evidence based best practice

– Utilize your skills in utilization management to ensure appropriate utilization of resources

– Collaborate with primary care physicians and other healthcare providers to ensure continuity of care

– Apply your knowledge of managed care principles to ensure cost-effective care delivery

– Utilize your expertise in case management to coordinate services across various healthcare settings

Skills:

– Valid RN license in Maryland

-Minimum of two years of experience in in a DDA or related setting preferred.

– 2 Years Working with Development Disabilities through the state of Maryland

– Strong knowledge of patient assessment techniques and vital signs monitoring

– Proficient in utilizing electronic medical records systems

– Excellent communication and interpersonal skills

– Ability to work collaboratively as part of a multidisciplinary team

– Strong organizational and time management skills

DDA Assistant Case Manager

Job description

Under the direction of the Case Manager the Assistant Case Manager will provide comprehensive case management services to individuals with intellectual Disabilities/Autism in the agency’s Meaningful Day, Personal Supports, and Respite programs.

Education/Training/Experience Requirements:
• At least 3 years experience working with individuals with Developmental Disabilities/Autism
• Bachelor’s degree preferred in addition to 3 years of experience in a supervisory capacity.

RN DDA Case Manager

The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.

Essential Duties And Responsibilities
• Performs and coordinates the initial assessments and assessments of the patient’s status
• Documents patient case information within a database system
• Performs chart review/audits monthly or as needed
• Participates in monthly case conferences by providing information pertinent to patient’s needs/goals
• Partners with the Program Director in the development and review of the patient’s individualized coordination of the care plan
• Ensures that the patient’s medical needs are addressed; consults with the patient’s physicians as needed, coordinating plans for treatment, and advocating for the patient when necessary
• Promotes understanding of the medical factors affecting the target population
• Identifies and assists patient(s) in accessing entitlements, resources, information, and referrals for psychosocial needs
• Participates in Quality Assurance and Utilization activities, as directed
• Empowers patients in decision-making for care planning
• Maintains accurate and timely patient information, which is readily accessible for review and meets all requirements; assists in data collection for reporting/funding sources
• Fosters intra-facility and inter-facility work relationships to help accomplish goals; acts as a liaison between primary care providers, specialists, and/or patient
• Advocates on behalf of patients regarding the accessibility of services
• Follows State/County mandated guidelines for the nurse case management programs
• Participates in outreach activities to the entire population, as directed
• Recommends program/service changes to meet gaps in service in the community
• Performs other duties as assigned/necessary

Minimum Requirements
• Current RN licensure in state practicing
• At least one year of Case Management experience preferred
• Complies with all relevant professional standards of practice
• Participation and completion of Amergis’ Competency program applicable
• Current CPR if applicable
• TB questionnaire, PPD or chest x-ray if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Successful completion of new hire training as applicable to the job site
• Understand patient confidentiality and HIPAA requirements
• Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, and clients) via strong communication skills; proficiency in the English language is required
• Computer proficiency required
• Must be at least 18 years of age

DDA Program Coordinator

Under the general supervision of Case Manager, the DDA Program Coordinator is responsible for the coordination and delivery of DDA Day services (Day Habilitation, Employment Services, Community Development Services, Supported Employment, Employment Discovery and Customization, Vocational Services and Career Exploration Services, Personal Supports, Respite Care) to individuals with intellectual and developmental disabilities. The DDA Program Coordinator performs job duties in line with Marvelous Support Inc’s. DDA’s mission, vision, and philosophy.

Roles and Responsibilities:
• New Client Management and Cultivation
• Day Program Activity Planning and Logistics
• New Staff Training
• Individual Initial PCP Meeting and overall PCP Management including quarterly reports
• Staff Scheduling, Supervision, and Management including performance reports, incident reports, and weekly meetings.

Responsibilities are clearly laid out and explained in your employment contract upon hiring.

Qualifications(Education/Training Requirements):

· Bachelor degree from an accredited university in a human services field

· Must have a valid driver’s license with reliable transportation

· Be eligible to employed in the special needs industry including pass a background check, not be simultaneously employed at another agency, be physically able to manage the staff and clients according to state guidelines
